To become an On-line member all one needs to do is request it and pay the annual membership fee. There are two ways to do this:

1. Simply click on the following link to access a printable version in PDF format for a membership formulary. Fill it in and mail it to the address listed on the formulary. Allow three to four weeks to receive your access codes thus enabling you to receive your electronic Newsletter;
2. For a quicker registration, you may subscribe by using Pay Pal (company associated with E-Bay) or by credit card. Using this method, you will receive your access code within 12 hours following your request.
